Abstract Šećerna bolest je jedna od najčešćih kroničnih nezaraznih bolesti, a možemo je svrstati u
globalne epidemije jer broj oboljelih svakodnevno raste. Obzirom da je šećerna bolest bolest
kroničnog tipa, sa sobom nosi rizik razvoja kroničnih komplikacija na svim organskim
sustavima. Poboljšano praćenje glukoze pomoću FreeStyle Libre sustava sastavljenog od čitača
i senzora koji svakodnevno daje uvid u kretanja glukoze je novost u liječenju šećerne bolesti.
Cilj ovog istraživanja je istražiti utjecaj kontinuiranog praćenja glukoze pomoću FreeStyle
Libre sustava na regulaciju šećerne bolesti i kvalitetu života oboljelih.
Provedeno je presječno istraživanje u kojem je sudjelovalo 217 sudionika koji koriste sustav za
kontinuirano praćenje glukoze (FreeStyle Libre) i državljani su Republike Hrvatske. Podatci za
istraživanje su prikupljeni na temelju anonimnog anketnog upitnika koji je proveden putem
Google Forms upitnika. Ispitanicima se pristupilo putem društvene mreže Facebook u zatvorenoj
grupi FreeStyle Libre Hrvatska.
Rezultati istraživanja pokazali su da samo četiri sudionika ima šećernu bolest tip 2, a ostalih
213 šećernu bolest tip 1. Prosječno trajanje bolesti iznosi 15,6 godina, a prosječna duljina
korištenja FreeStyle Libre sustava 2,3 godine. Većina od četiri petine sudionika nema kronične
komplikacije šećerne bolesti. U liječenju šećerne bolesti, preko devet desetina sudionika
primjenjuje terapiju inzulinom, a preostali kombinaciju inzulina i peroralnih hipoglikemika.
Uređaj za kontinuirano praćenje glukoze FreeStyle Libre, je većina sudionika dobilo putem
HZZO (Hrvatskog zavoda za zdravstveno osiguranje), a indikacija je bila intenzivirana
terapija inzulinom s dokazanom hipoglikemijom u bolničkim uvjetima. Samo jedna ispitanica
je bila trudnica. Važan rezultat ovog istraživanja je podatak da je korištenje kontinuiranog
praćenja glukoze FreeStyle Libre gotovo svim ispitanicima omogućilo sigurniju primjenu
terapije, doprinjelo većoj samokontroli vezanoj za samu bolest te se ne bi vratili mjerenju šećera
konvencionalom metodom. Također, ne manje važni rezultati ovog istraživanja su da
upotreba (FreeStyle Libre) sustava za kontinuirano praćenje glukoze ima pozitivan utjecaj na
kontrolu glikemije ograničavajući glikemijsku varijabilnost i smanjujući učestalost
hipoglikemijskih epizoda te dugoročno poboljšava kontrolu glikemije mjerenu glikiranim
hemoglobinom (HbA1c).
Na svim domenama upitnika za fizičko zdravlje, psihološko zdravlje i socijalne odnose
sudionici postižu prosječne rezultate koji ukazuju na umjereno zadovoljstvo. Na pitanju kojim
se procjenjuje ukupna kvaliteta života sudionici postižu visoke rezultate, a na pitanja kojim se
procjenjuje ukupno zadovoljstvo zdravljem, umjereno visoke rezultate. Najnegativnije
rezultate postižu u odgovorima na pitanje „Koliko često doživljavate negativne osjećaje kao
što su loše rasploženje, očaj, tjeskoba, potištenost?“ Također, u našem istraživanju pronađene
su male, no statistički značajne pozitivne povezanosti između duljine korištenja FreeStyle Libre
sustava i psihičkog zdravlja. Rezultati ovog istraživanja potvrđuju da korištenje i veća
učestalost skeniranja glukoze pomoću FreeStyle Libre pozitivno korelira s ponašanjem i
kvalitetom života bolesnika oboljelih od šećerne bolesti.
Abstract (english) Diabetes is one of the most common chronic non-communicable diseases, and we can classify
it as a global epidemic because the number of patients is growing every day. Because diabetes
is a chronic type, it carries with it the risk of developing chronic complications in all organ
systems. New in the treatment of diabetes is improved glucose monitoring using the FreeStyle
Libre system composed of a reader and sensor that provides daily insight into glucose
movements. The aim of this study was to investigate the impact of continuous glucose
monitoring using the FreeStyle Libre system on the regulation of diabetes and the quality of life
of patients.
A cross-sectional study was conducted in which 217 participants used the FreeStyle Libre
system and are citizens of the Republic of Croatia. Survey data were collected based on an
anonymous survey questionnaire conducted through the Google Forms questionnaire.
Respondents were accessed via the social network Facebook in the closed group FreeStyle Libre
Croatia.
The results of the study showed that only four participants had type 2 diabetes and the other
213 had type 1 diabetes. The average duration of the disease was 15.6 years, and the average
length of use of the FreeStyle Libre system was 2.3 years. Most of the four-fifths of participants
do not have a chronic complication of diabetes. In the treatment of diabetes, over nine-tenths
of participants administer insulin therapy, and the remaining a combination of insulin and
tablets. Most participants received the FreeStyle Libre continuous glucose monitoring device
through the Croatian Institute for Health Insurance, and the indication was intensive insulin
therapy with proven hypoglycemia in a hospital setting. An important result of this study is
that the use of continuous glucose monitoring FreeStyle Libre allowed almost all subjects to use
the therapy more safely, contributed to greater self-control related to the disease itself and
would not return to measuring sugar by conventional methods. Also, no less important results
of this study are that the use of (FreeStyle Libre) continuous glucose monitoring system has a
positive effect on glycemic control by limiting glycemic variability and reducing the frequency
of hypoglycemic episodes and long-term improved glycemic control measured by glycated
hemoglobin (HbA1c).
In all domains of the questionnaire, participants achieve average results that indicate moderate
satisfaction. On the question of which the overall quality of life is assessed, the participants
achieve high results, and on the question of which the overall health satisfaction is assessed,
moderately high results. The most negative results are achieved in the answers to the question
"How often do you experience negative feelings such as bad mood, despair, anxiety,
depression?" The results of this study confirm that the use and higher frequency of glucose
scans using FreeStyle Libre is positively correlated with the behavior and quality of life of
patients with diabetes.
