Abstract UVOD: Opseg intervencija hitne medicinske pomoći obuhvaća i hitne slučajeve povezane s palijativnom skrbi. Intervencijom hitne pomoći može se značajno smanjiti patnja pacijenata s intenzivnim simptomima, pružajući ključnu podršku u kritičnim trenucima kako bi se pacijentu osigurala dostojanstvena smrt.
CILJ: Cilj istraživanja bio je ispitati znanja i stavove medicinskih sestara/tehničara Hitne medicinske službe u Šibensko-kninskoj županiji o palijativnoj skrbi.
METODE: Istraživanje je provedeno u Hitnoj medicinskoj službi Šibensko Kninske županije od siječnja do listopada 2022. godine. Ispitanici su bili medicinske sestre i medicinske tehničari koji su zaposleni u navedenoj Hitnoj medicinskoj službi Za potrebe ovog istraživanja provedena je anonimna anketa koja se sastojala od tri dijela. Prvi dio je ispitivao sociodemografska obilježja ispitanika, drugi dio je ispitivao znanje ispitanika o palijativnoj skrbi i sastojao se od 10 odabranih tvrdnji iz upitnika Palliative care Quiz for nurses (PCQN), dok se treći dio upitnika sastojao od pitanja koja se odnose na mišljenja i stavove ispitanika o palijativnoj skrbi, a koja su samostalno pripremljena za potrebu ovog istraživanja. U dijelu upitnika koji je ispitivao znanje, rezultat od 75% točnih odgovora se smatra kao „dobro znanje“ dok je 50% točnih odgovora „dostatno znanje“.
REZULTATI: U istraživanju je sudjelovalo 54 ispitanika, od kojih je 36 žena i 18 muškaraca. Znanje ispitanika o palijativnoj skrbi je iznimno loše, jer je samo jedan ispitanik uključen u istraživanje imao ukupno 80% točnih odgovora, a njih 20 (37,03%) imalo je „dostatno znanje“. Najviše točnih odgovora, odnosno njih 61,11% bilo je za tvrdnju „Patnja i fizička bol su jedno te isto.“ Na tvrdnju „Osobe koje uzimaju opioide moraju uzimati i laksative“ točno je odgovorilo 55,56% ispitanika. Od ukupnog broja ispitanika njih 51,85% točno je odgovorilo na tvrdnju „Muškarci se lakše nose sa tugom od žena.“. Iako rezultat ne prelazi 50%, najveći broj od 40,74% netočnih odgovora zabilježen je na tvrdnji „Opseg bolesti određuje način liječenja boli“. Većina ispitanika ima pozitivno mišljenje o skrbi za palijativne bolesnike od strane hitne medicinske pomoći. Znanja i stavovi ispitanika o palijativnoj skrbi nisu u korelaciji sa stupnjem obrazovanja i duljinom radnog staža.
ZAKLJUČAK: Rezultati ovog istraživanja pokazuju nedostatnost znanja o palijativnoj skrbi kod medicinskih sestara i medicinskih tehničara u Hitnoj medicinskoj službi Šibensko -Kninske županije čime se dovodi u pitanje i kvaliteta provođenja iste u praksi. Stoga, rezultati ovog istraživanja ukazuju na potrebu za dodatnom edukacijom medicinskih sestara i tehničara iz područja palijativne skrbi, čime bi se najvjerojatnije i poboljšala kvaliteta palijativne skrbi.
Abstract (english) INTRODUCTION: The scope of emergency medical care interven*tions includes emergency cases related to palliative care. Emergency intervention can significantly reduce the suffering of patients with intense symptoms, providing crucial support at critical moments.
OBJECTIVE: The general objective of the research was to examine the knowledge and attitudes of nurses/technicians of the Emergency Medical Service in Šibenik-Knin County regarding palliative care.
METHODS: The research was conducted in the Emergency Medical Service of Šibenik-Knin County from January to October 2022. Respondents were nurses and medical technicians who are employed in the Emergency Medical Service. For the purposes of this research was an anonymous survey, that consisted of three parts, conducted. The first part examined the sociodemographic characteristics of the respondents, the second part examined the knowledge of the respondents about palliative care and consisted of 10 selected statements from the Palliative care Quiz for nurses (PCQN), while the third part of the questionnaire consisted of questions related to opinions and attitudes respondents about palliative care, which were independently prepared for the needs of this research. In the part of the questionnaire that examined knowledge, a result of 75% correct answers is considered as "good knowledge", while 50% correct answers is "sufficient knowledge".
RESULTS: 54 respondents participated in the research, 36 were women and 18 were men. The respondents' knowledge about palliative care is extremely poor, because only one respondent included in the research had a total of 80% correct answers, and 20 of them (37.03%) had "sufficient knowledge". The most correct answers, or 61.11% of them, were for the statement "Suffering and physical pain are one and the same." The statement "People who take opioids must also take laxatives" was answered correctly by 55.56% of respondents. Out of the total number of respondents, 51.85% correctly answered the statement "Men cope with sadness more easily than women." Although the result does not exceed 50%, the highest number of 40.74% of incorrect answers was recorded on the statement "The extent of the disease determines the method of pain treatment". The majority of respondents have a positive opinion about the care of palliative patients by emergency medical services. The respondents' knowledge and attitudes about palliative care are not correlated with the level of education and length of service.
CONCLUSION: The results of this research show insufficient knowledge of palliative care among nurses and medical technicians in the Emergency Medical Service of the Šibenik-Knin County, which calls into question the quality of its implementation in practice. Therefore, the results of this research indicate the need for additional education of nurses and technicians in the field of palliative care, which would most likely improve the quality of palliative care.
